Identify specific actions to achieve health goals Explain reasons behind our chosen actions Recognize barriers that might hinder our progress Identify enablers that will support our success Create realistic timeframes for our actions
Success Criteria
I can identify at least 2 specific actions for my health goal I can explain WHY each action will help me reach my goal I can list 2-3 barriers for each action I can identify 2-3 enablers for each action I can set realistic timeframes (days, weeks, months) I can present my plan clearly to others
Think About It
What does a good action plan look like? Think of a time you successfully achieved a goal - what helped you?
What Makes an Effective Action?
Specific - clearly defined, not vague Measurable - you can track progress Achievable - realistic for your situation Relevant - directly helps your goal Time-bound - has a clear deadline

Work in pairs to complete the planning template Choose a realistic health goal (fitness, nutrition, sleep, stress management) Complete BOTH actions with full details Be specific about timeframes and reasons
